What is Dialectical Behavior Therapy (DBT)?

Dialectical behavior therapy is a specialized form of psychotherapy (talk therapy) that was developed by Marsha Linehan, PhD, in the 1970s and 1980s. 

Dr. Linehan originally created DBT in an attempt to find a more effective approach for women who had borderline personality disorder (BPD) and persistent suicidal ideation. In addition to her considerable professional training, Dr. Linehan also brought significant personal experience to this effort, as she also has BPD.

Though DBT’s origins are tied to borderline personality disorder and suicidal ideation, this approach has also proved to be beneficial for people with a range of additional mental and behavioral health concerns, including:

The 4 Components of DBT

Dialectical behavior therapy is a structured approach that is designed to help participants improve their skills in four areas:

  1. Mindfulness: This practice encourages people to remain fully present in the moment, and to maintain a non-judgmental mindset when considering their own thoughts and emotions, their personal experiences, and the actions of others.
  2. Emotion regulation: This skill area focuses on helping participants recognize and accept their emotions, understand the relationship between their emotions and their behaviors, and exert greater control over how they express their emotions.
  3. Distress tolerance: During distress tolerance sessions, participants learn how to cope with painful experiences and live with difficult emotions without attempting to avoid them or impulsively reacting in an unhealthy manner.
  4. Interpersonal effectiveness: This helps patients apply the skills they developed in the other three areas to build healthy relationships, establish and maintain appropriate boundaries, and resolve conflicts in a productive manner. 

DBT is typically provided via both individual and group sessions. Some providers also offer phone or online consultations between sessions.
